Urgent Requests
If your patient’s needs are urgent, please call us so we can accommodate a SAME DAY or NEXT DAY appointment.
Online Referral
Online referrals will be sent securely directly to our office, and we will call your patient to schedule their appointment as soon as possible.
Referral by FAX
Please fax appointment requests along with recent office/progress note, demographics, insurance information and referral/authorization if required.
Custom Fax Form
Please let us know if you’d like to request a personalized appointment request form, which includes your practice information.

EMR
If you wish to send a referral directly from your EMR, please do! Some helpful information to include is the most recent office / progress notes, demographics, insurance information, and referral / authorization if required. We are also available on eCW P2P.
If you have email encryption capability to send a referral via your own secure email, you are welcome to send referrals by email to appointments@oracleheartva.com.
